I agree to indemnify and hold harmless The SF Mushroom Collective, James McConchie, Niko Summers and Stefan Kasian the releasees from any loss, liability, damage or cost from any services rendered. I attest that all the information below is truthful and up to date. I promise to legally share and consume entheogens in accordance with San Francisco City resolution # 220896. The consultation services of the SF mushroom Collective are solely educational, and I will not participate in any illegal activity involving entheogens and/or psychedelics.
